Viagra and Cialis and Trimix Oh My!!!!

20160406_130430I make an effort to always have fresh flowers in our house for my wife to enjoy.  After just two days in the vase, it’s easy to see which flowers have erectile dysfunction in this picture.

Erectile dysfunction is one of the services available with the Medicine Coach membership program.  As men age, many notice some difficulties in the bedroom.  There is both difficulty achieving an erection as well as difficulty maintaining an erection.  Both issues can be solves easily.

Let’s just briefly explain how the two most popular erectile dysfunction medications work.  When stimulated, an enzyme in the penis produces messenger cyclic guanosine monophosphate (cGMP).  cGMP relaxes smooth muscle causing more blood to flow into the penis.  Phosphodiesterase 5 (PDE-5) breaks down cGMP.  Viagra (sildenafil) and Cialis (tadalafil) block PDE-5.  This results in more cGMP, meaning more blood flow, meaning erection.

Both sildenafil and tadalafil are available from my clinic in a rapidly dissolving tablet.  The tablet dissolves under your tongue or between your cheek and gum in about 30 seconds.  Sildenafil begins to work in 15 to 20 minutes and lasts two to three hours.  Tadalafil begins to work in 45 to 60 minutes but lasts two to three days.

If you are one of the small percentage of men who do not respond to Viagra or Cialis, there are other options.  An injectable medication called TriMix can be used for these men.  As horrifying and terrible as it sounds, injecting this medication into the side of your penis is less painful than you would imagine.  For many men, the short period of discomfort is well worth the result.

TriMix is a combination of papaverine, phentolamine, and alprostadil.  Papervine relaxes smooth muscle, phentolamine directly causes blood vessels to expand, and alprostadil works both directly and indirectly to dilate blood vessels.  The result is an erection beginning 7-8 minutes after injection.  Stimulation is not needed in this case.

The starting dose of TriMix is literally one drop, injected with an insulin syringe.  You slowly increase dose until you get your individual desired result.  I tell guys, the more you inject, the longer you will maintain an erection, but be cautious.  All my clients are required to have the antidote on hand.  Having a painful or 4-hour erection is no joke and is not pleasurable.  Without the antidote, an embarrassing trip to the emergency room will be the result of too much “fun.”
